Appointments

To ensure that you get an appointment for the date and time you desire, we suggest booking all appointments a few weeks in advance. Our Website displays the most up-to-date availability. Please note that we require a credit card to reserve your appointment. We accept American Express, Visa, MasterCard, and Discover for booking an appointment.

Cancellations

As a courtesy to our providers, for non-group appointments, please provide a minimum of 24 hours notice should you need to cancel or reschedule an appointment. You will be charged $75 if an appointment is canceled or rescheduled less than 24 hours in advance or if there is a no-show.
For group appointments, there is a 72-hour cancellation policy, and the credit card on file will be charged $100 if canceled less than 72 hours in advance.
If you arrive more than 10 minutes late for your appointment, you may be required to reschedule to avoid disrupting other clients’ appointments.
There are no exceptions to these policies.

Returns & Refunds

If you would like to return your purchased skincare product, return the item in its original packaging unopened within 30 days of purchase. We do not accept returns on opened or used products. We do not accept returns or exchanges on gift cards. To request a refund or exchange, email info@vitalizewellnessnc.com and include your receipt.
Services received can not be refunded.
Packages are non-refundable

Consultations

We provide free consultations and in-depth skin analysis with one of our expert providers. We believe an informed client with realistic expectations will be most satisfied with our treatments and results. Our consultations include education about your unique skin type, a comprehensive discussion about all options available to you, both immediate and long term, and a clear plan of action based on your individual goals.
If you arrive more than 10 minutes late for your appointment, you may be required to reschedule to avoid disrupting other clients’ appointments.
There is a limit of 2 free consultations per person per 365 days.

Referrals

If you refer a friend or family member to Vitalize Wellness, you will receive a $40 credit toward your next service. All referral credits will be issued to you once the client is treated.

Film & Photo Policy

Clients, or a friend of a client, are prohibited from filming, recording, or taking photos of a Provider or Aesthetician in a treatment room or anywhere on Vitalize Wellness property. If you would like to film, please seek written approval from management.

WHO WILL FOLLOW THIS NOTICE?

This notice describes the practices of Vitalize Health PLLC, DBA Vitalize Wellness, and the practices that will be followed by all Vitalize Health PLLC, DBA Vitalize Wellness workforce members who handle your medical information.

OUR PLEDGE REGARDING YOUR PROTECTED HEALTH INFORMATION

Vitalize Health PLLC, DBA Vitalize Wellness understands that medical information about you and your health is personal. We are committed to protecting medical information about you. We maintain our records and conduct our treatment environment to provide a robust level of protection for your medical information, while still providing you with a high level of medical care. This notice applies to all of the records of your medical care that are received or created by Vitalize Health PLLC, DBA Vitalize Wellness.

Your other medical treatment providers (e.g. doctors, nurses, hospitals, home health agencies, etc.) may have different policies or notices regarding the use and disclosure of your medical information.

This notice will tell you about the ways in which Vitalize Health PLLC, DBA Vitalize Wellness may use and disclose medical information about you. Your medical information, also referred to as “protected health information” is that information about you, including demographic information, that may identify you and that relates to your past, present, or future physical or mental health information and related health care services.

In this notice, we also describe your rights and certain obligations Vitalize Health PLLC, DBA Vitalize Wellness has regarding the use and disclosure of your protected health information. We are required to:

  1. Make sure that medical and other information that identifies you (protected health information) is kept private.

  3. Follow the terms of the notice that is currently in effect.
  4. To request your records, please send Vitalize Health PLLC, DBA Vitalize Wellness a written request for your medical records.  

USES AND DISCLOSURES FOR TREATMENT, PAYMENT AND HEALTH CARE OPERATIONS

By becoming a patient at Vitalize Health PLLC, DBA Vitalize Wellness, you are giving consent for Vitalize Health PLLC, DBA Vitalize Wellness to use your protected health information for certain activities, including treatment, payment and other health care operations. 

We may use and disclose protected health information about you so that Vitalize Health PLLC, DBA Vitalize Wellness, and its medical professionals can treat you. For example, we may use your past medical information to diagnose your present condition or we may provide information regarding your medical condition to another doctor to whom we refer you for additional care. We may also use and disclose protected health information about you so that we may be paid for the medical treatment we provide you.

We may also use and disclose protected health information about you for Vitalize Health PLLC, DBA Vitalize Wellness health care operations, in other words, those other tasks that we need to perform to make sure that you are provided a high-quality of medical care. For example, we may use your protected health information to evaluate how we can better meet your needs.  We may also share your protected health information with third parties in connection with services that we provide or if required by law or upon regulatory request.

Other uses and disclosures of your protected health information

The following uses of your protected health information may be made without any additional authorization from you. (Not every use or disclosure is listed, but be assured that all uses and disclosures made by Vitalize Health PLLC, DBA Vitalize Wellness are only those which are permitted under the law).

Uses and disclosures for appointment reminders. Furthermore, any pictures, stories, letters, biographies, correspondence, or thank you notes sent to us become the exclusive property of Vitalize Health PLLC, DBA Vitalize Wellness. We reserve the right to use non-identifying information about our clients (those who receive services or goods from or through us).

Changes to the Terms of this Notice:

We can change the terms of this notice, and the changes will apply to all information we have about you. The new notice will be available upon request, in our office, and on our web site.

European Union Data Transfer

We are based in the United States. By using our services, you consent to transfer your personal data to the United States and to the processing and use of that personal data in the United States. This processing and use are limited to what is described in this privacy policy and the terms of use.
We may use your information to offer you products and services that we think may interest you unless you have opted out of marketing communications. If you are in the European Economic Area, where required by applicable law, we will obtain your consent before sending you any marketing communications. You may object to using your data for direct marketing purposes at any time, free of charge, when we collect your personal information, and in every marketing communication. We will never provide your information to third parties for marketing purposes without your prior consent.
To ensure the best experience for all Vitalize Wellness clients and team members, clients who exhibit disruptive or aggressive behavior or have a history of no-showing appointments may be denied the opportunity to schedule treatments.
