Vitalize Wellness Cancellation Policy

Vitalize Wellness has instituted an Appointment Cancellation Policy. A cancellation made with less than a 24-hour notice significantly limits our ability to make the appointment available for another patient in need. To remain consistent with our mission, we have instituted the following policy:

Please provide our office with a 24-hour notice in the event that you need to reschedule your appointment. This will allow us the opportunity to provide care to another patient. A voicemail or text message can always be left to avoid a cancellation fee being charged.

A “No-Show”, “No-Call” or missed appointment, without proper 24-hour notification, may be assessed a $75 fee.

Please note that we will charge a $75 dollar fee or the total amount of your appointment (ex: if you have a B12- shot for $20 and you don’t show we will charge the $20 not the $75) for cancellations or no shows that are made within 24hrs of your appointment. If you need to reschedule your appointment within the 24rhs of your appointment you will be charged a late reschedule fee of $25 dollars.

If you are 15 or more minutes late for your appointment, the appointment may be canceled and rescheduled if we have not heard from you.

There will be no charge for appointments that are canceled or rescheduled with 24 hrs notice.

Appointments must be canceled at least 24 hours prior to the scheduled appointment time.

Repeated missed appointments may result in termination of the physician/patient relationship.
