This is a recurring monthly membership and will be charged to your credit card automatically. Six months minimum. Forfeit the cost of one month if you cancel early. It will not stop after 6 months but will continue to run until YOU cancel it. Cancel at anytime after 6 months with a 30 day written notice to info@vitalizewellnessnc.com or text 919-739-2668. NO REFUNDS. Purchased services are not shareable. Monthly account credit rolls over, but will expire in 1 year.

If you don't reschedule your appointment with 24hrs notice your monthly service will be forfeited. No exceptions!
If you don't show/call to your booked appointment you lose your the schedule treatment for the month.
If you cancel your membership before the 3 months commitment period you will be charge a $100 cancellation fee.
